DOG LICENSES
All dogs over the age of six months must be licensed. The fee is $15 for each dog or $7 for a dog that has been spayed or neutered. Special licenses are required for Breeders. Please contact the Town Clerk's Office for details. Residents must submit copy of the Rabies Vaccination Certificate and Spaying or Neutering Certificate with appropriate fee to Town Clerk's Office in order to obtain a license.

MARRIAGE LICENSES
The bride and groom both must be present in order to apply for a license. The Bride and Groom are required to have the license at least 24 hours before the wedding can take place, but no more than 60 days in advance with the exception of an active military applicant in which case can be up to 180 days in advance. Bring a photo I.D. (Driver's License, Non-Driver's License, Passport, Employment picture ID or Immigration record) and a record showing the date of birth, place of birth and parents' names (certified copy of their birth certificate, baptismal record, naturalization record or census record.) If this in not eithers first marriage, the certified divorce papers or relevant death certificates of all previous marriages must be shown at the time of application. The license is $40.00. The Town of Herkimer only accepts Cash and Checks.

Marriages in which either party is under the age of seventeen years is prohibited. Issuance of a Marriage License to a minor at least seventeen years of age but under eighteen years of age requires approval of both parents (or one parent if conditions are met) and written approval of a justice of the supreme court or of a judge of the family court, having jurisdiction over the town or city in which the application is made.

COPIES OF MARRIAGE, BIRTH OR DEATH CERTIFICATES
Certified copies of Marriage Licenses that had previously been applied for, through this office, can be obtained and be issued to only the person named on the document. The fee is $10.00 per copy. An application must be filled out and you must have a valid photo ID.

Birth and Death Certificates are available as well. Birth Certificates are only issued to the person named in the document; Death Certificates are issued to members of the immediate family. Birth and Death Certificates are available through the town if the birth or death occurred in the home. Records may be applied for in person or by mail with the applicant providing Photo I.D. The fee for Birth or Death records is $10.00 per copy.

HANDICAPPED PARKING PERMIT
Applications for residents of the Town of Herkimer and the Village of Herkimer are available at the Town Clerk's Office. The applicant and his/her physician must complete the forms. Please bring in a valid driver's license for proof, since New York State requires the last three digits of the driver's license on the permit. The application is then returned to this office and a Permanent Permit is issued for a period of five years. Temporary Permit is issued for up to six months. The permit is valid in New York State and is recognized in many other states as well.

COPIES OF MINUTES, LOCAL LAWS, ETC.
Copies of minutes of Town Board, Planning Board and Zoning Board of Appeal meetings are available in the Town Clerk's Office for a fee of $.25 per page. Zoning Books and Maps are available for purchase at $8.00 each or $15 for both. They are also available on our website for viewing or printing.

REGISTERING TO VOTE
U.S. citizens, 18 years of age and older, are eligible to vote provided they are properly registered. All voter registration is done by mail. Forms are available at the Town Clerk's Office and may be picked up in person or sent upon request. The registration form must be completed and mailed to the Herkimer County Board of Elections, 109 Mary Street, Suite 1306, Herkimer, NY 13350. ABSENTEE BALLOTS
Applying for an Absentee Ballot is a two-step process. Application forms are available from the Board of Elections or the Town Clerk's Office and must be completed and returned to the Herkimer County Board of Elections. The Board will then send the ballot to the applicant. The address of the Board of Elections is Herkimer County Board of Elections, 109 Mary Street, Suite 1306, Herkimer, NY 13350.
